Social Prescribing

What is social Prescribing?

Social Prescribing is a non-medical non-Urgent referral option for patients

  • With one or more long term conditions
  • Who need support with their mental Health
  • Who are lonely and isolated
  • Who have complex social needs which affect their wellbeing

Patients can be referred to social prescribing for any of the following issues:

Benefits/Finance/form filling

Bereavement support

Carers Services

Social Activities/Community Groups

Counselling Options

Emotional Wellbeing support

Social Isolation/Loneliness

Domestic abuse referrals

Advocacy support

Housing support

Weight/Lifestyle changes

Volunteering/Training opposrtunities 

Who can refer?

Anyone can refer to me, including self-referral  as long as you are a patient registered at one of the surgeries listed below.

  • Dengie Medical Partnership
  • William Fisher medical Centre
  • The Trinity medical Practice
  • The Burnham Surgery

SOUTHMINSTER MEN’S SHED

SOUTHMINSTER MEN’S SHED

Men’s Sheds are community spaces for men to connect, converse and create. The activities are often similar to those of garden sheds, but for groups of men to enjoy together. They help reduce loneliness and isolation, but most importantly, they’re fun.

 

Southminster Men’s Shed, which is hosted by Maldon & District CVS, was founded in 2023 and meets at the Station House, Southminster Station.  Sessions are currently Monday's 9-12pm and 12:30-3:30PM, Wednesday's 9-12pm and Friday's 9-12pm.
