New Patient Registration

To register as a patient ask at reception for details where possible between 4pm and 6pm or use the online registration link. New patients will be invited to make an appointment to discuss their health needs and an initial appointment will be made with our health care assistant for this purpose.

In the interests of continuity of care, the doctors in this practice have individual patient lists. However, when registering with the practice, where possible patients' choice of doctor will be accommodated.

The practice area covers Burnham-on-Crouch, Althorne and North Fambridge.  Burnham Surgery Catchment Area

Burnham surgery encourages patients to see their usual doctor for ongoing problems, this allows for continuity of care. If your request is urgent then you may be seen by any of the General Practitioners that work in the surgery.

Please note the appointment time given for a GP consultation is 10 minutes. It can be longer for more complicated reviews and procedures. If you have a significant problem that will require more time please inform the receptionist at the time of your booking request. These are not usually for urgent requests and you may have to wait longer for appointment availability.

Medical treatment is available from the date of acceptance of registration. Please contact reception for further information.

ONLINE NEW PATIENT REGISTRATION

The registration process will take around 15 minutes to complete. You will need to complete the process in one session. You do not need your NHS number, but it could make registration easier.

We may ask you for:

  • details of previous GP surgery
  • basic health and medical information

The information you provide will be used to identify the correct medical records. It will also help the GP surgery to offer relevant and appropriate healthcare services.

2. New Patient Questionnaire

Either bring to surgery in person or email signed and completed forms to discharge.f81126@nhs.net 

When attending the practice in person with your forms please try an attend between 11am - 12 noon or 4pm - 6pm when reception will hopefully be less busy.

Do not know your NHS Number?

You should be able to find your NHS Number on any letter or document you have received from the NHS, including prescriptions, test results, and hospital referral or appointment letters. You can also find your NHS Number online via the https://www.nhs.uk/nhs-services/online-services/find-nhs-number/

If you cannot find your NHS Number at home, you can ask your GP practice that you are leaving from should be able to help you. You may find your NHS number on your repeat slip or hospital letters.
