New Patient Registration
To register as a patient ask at reception for details where possible between 4pm and 6pm. New patients will be invited to make an appointment to discuss their health needs and an initial appointment will be made with our health care assistant for this purpose.
In the interests of continuity of care, the doctors in this practice have individual patient lists. However, when registering with the practice, where possible patients' choice of doctor will be accommodated.
The practice area covers Burnham-on-Crouch, Althorne and North Fambridge.
Burnham surgery encourages patients to see their usual doctor for ongoing problems, this allows for continuity of care. If your request is urgent then you may be seen by any of the General Practitioners that work in the surgery.
Please note the appointment time given for a GP consultation is 10 minutes. It can be longer for more complicated reviews and procedures. If you have a significant problem that will require more time please inform the receptionist at the time of your booking request. These are not usually for urgent requests and you may have to wait longer for appointment availability.
Medical treatment is available from the date of acceptance of registration. Please contact reception for further information.

3. Summary Care Consent
4. Online access
and email with scanned proof or picture proof of ID and Address - during covid 19 to email@example.com or when able to do so safely bring in proof of address (utility bill) and photo ID (passport / drivers licence)
When attending the practice in person with your forms please try an attend between 11am - 12 noon or 4pm - 6pm when reception will hopefully be less busy.
