New Website Launched to Provide Local Health and Wellbeing Information
We’re proud to launch our new Dengie and South Woodham Ferrers Primary Care Network website together with our partners in the network. You can find the new website at www.dengieandswfpcn.nhs.uk.
This new website will support patients to self-care by providing accessible information and advice on a range of topics, including mental health support, weight management, cancer services and more. All the information has been reviewed by clinicians and residents to ensure it meets their needs.
This website has been designed for our local population, and will help patients improve their health, wellbeing and social welfare by connecting them to the full range of services available in our community. This includes support available from the voluntary and charity sector, local authority and other partners.

Primary Care Networks offer a new way of organising care, bringing together all local health and care services in a local area to deliver more joined up care to local populations (usually covering around 50,000 people).
